4) List all of your computer skills and proficiencies (IT people - list your technical proficiencies also). Please think hard, listing only those skills which you can use competently. If you excel in any particular areas, please inform us.

5) Have you received any awards, including military certificates, or recognition for performance in your work? What about academic awards? What about extracurricular activities and volunteer experiences?

6) Are you conversant in any other language(s) besides English? What is your level of expertise (for each foreign language)?

7) Explain in detail (in simple words that anybody can understand) what major accomplishment you've undertaken on the job of which you are the most proud. Then, explain how you did it!

8) Military service? List Highest Rank, Years, Titles, Locations, City, State and Country. For career military, include with history of employment.
